Your Member Handbook informs you of your rights as a member. It also explains your benefits, how to access care, and more. It’s specific for where our members live. You can view or download the most recent handbook below. If you need a handbook in large print, in a language other than English, or in a different format, let us know by calling (800) 431-4135.
We want to help you get rides to and from your doctor’s appointments. We work with agencies to help you get to your healthcare appointments. The program is called Non-Emergency Medical Transportation (NEMT).
Non-Emergency Medical Transportation is how you can get a ride to a covered healthcare appointment. This is for scheduled health care appointments, not emergencies. There are many ways we can help you get to your appointment. Examples are:

PacificSource Community Solutions operates the Coordinated Care Organizations (CCO) that serve members of the Oregon Health Plan in Central Oregon. Each Coordinated Care Organization has its own Community Advisory Council (CAC), made up of members like you, providers, and other community members.
Our Councils provide advice and recommendations to us about member and community needs. They also advise us about how to respond to members’ needs and plan for community health. They make recommendations about preventive care and strategic planning.
Our Councils also work to improve the service that PacificSource Community Solutions and our provider organizations offer members. They identify opportunities to improve and make suggestions about our programs.
The Councils are your voice in the health plan. Most Council members (more than half) are PacificSource Community Solutions members. The Councils give you the chance to take an active role in improving your own health and that of your family and community members.
The Councils also oversee a Community Health Needs Assessment and a Community Health Improvement Plan.
