Effective 8/2/2021, prior authorization requirements will be removed for PT/OT/ST and Alternative Services. As a reminder, prior authorizations are always required for out-of-network providers.

We acknowledge there have been previous communications regarding PT/OT/ST and Alternative Services prior authorization requirements, this notice supersedes any prior notices. Should you have any questions or claims impacted please contact your Provider Service Representative.

*Please Note- Prior Authorization Requirements are reviewed continuously and can be changed at any time.  Should PA requirements be turned back on for these services, a Provider Notice will be posted giving 30 day advanced notice.
